Indications
A drug called Cytomis 200 is used to induce medical abortions (terminating a pregnancy). Additionally, it is employed to stop excessive postpartum bleeding. The uterus contracts more as a result of this medication's action. To get the most benefit, Cytomis 200 is administered either alone or in conjunction with another medication. It should be taken with food or as directed by your doctor.
Pharmacology
Misoprostol is rapidly de-esterified to its free acid, which is responsible for its clinical activity and, unlike the parent compound, is detectable in plasma. This free acid is responsible for misoprostol's extensive absorption. When taken with food, misoprostol acid doses result in lower maximum plasma concentrations, and taking an antacid at the same time decreases the availability of misoprostol acid overall. Misoprostol has mucosal protective properties in animals as well as antisecretory properties that prevent the secretion of gastric acid. A lack of prostaglandins within the gastric mucosa may result in dwindling bicarbonate and mucus secretion and may contribute to the mucosal damage brought on by NSAIDs, which inhibit prostaglandin synthesis. Misoprostol can increase the production of bicarbonate and mucus in humans, but this has only been demonstrated at doses of 200 and higher, which are also antisecretory.
Dosage & Administration
Interaction
There is no evidence of clinically significant interaction between Misoprostol and cardiac, pulmonary, CNS drugs and NSAIDs. The bioavailability of Misoprostol is decreased with high doses of antacid.
Contraindications
Misoprostol is contraindicated to anyone with a history of allergy to prostaglandins and it is also contraindicated in pregnancy.
Side Effect
Generally, Misoprostol is well tolerated. The most frequent adverse effects associated with Misoprostol therapy involve the GI tract such as diarrhea, abdominal pain, dyspepsia, flatulence, nausea, vomiting, rashes, and dizziness. The incidence of diarrhea may be minimized by administering the drug after a meal and at bedtime and by avoiding concomitant administration with a magnesium-containing or other laxative antacid.
Pregnancy & Lactation
Misoprostol's component is contraindicated in pregnant women due to its abortifacient properties. Unless the patient requires nonsteroidal anti-inflammatory drug (NSAID) therapy and is at a high risk of developing gastric or duodenal ulceration or for developing complications from gastric or duodenal ulcers associated with the use of the NSAID, it shouldn't be administered to women who are or may become pregnant. Although it hasn't been researched, it's possible that the active metabolite (misoprostol acid) is excreted in milk. It is not advised for use by nursing mothers due to the possibility of serious adverse reactions in nursing infants.
Precautions & Warnings
Misoprostol is contraindicated in pregnant women and should not be used in women of childbearing potential unless the patient requires NSAID therapy in the prevention and treatment of NSAID-induced gastric and duodenal ulcers. It is important to inform women of childbearing potential that they must not be pregnant when misoprostol therapy is started and that they must use an effective method of contraception while taking the medication.
If labor must be induced, the pregnancy must have reached 36 weeks' gestation with a maternal or fetal medical indication, or it must have reached 38 weeks' gestation by reliable dating, or it must have reached lung maturity as shown by an L/S >2.0 or a positive phosphatidyl glycerol test, or it must have reached 36 weeks gestation. Acute fetal distress, placenta abruption, placenta previa, or unexplained vaginal bleeding are contraindications to inducing labor. The fetus should present with its vertex.
